Alternative Care Management

 

The key focus of the HFHK Alternative Care Management Program is to ensure the continued success of the alternative care arrangements established in HFHK's Alternative Care Development Program and to monitor the health, well being and overall development of the children who have been deinstitutionalised. 


HFHK has now closed the two children's homes under our care.  As a result, 21 children have been re-integrated with their families, 6 children are now living independently and a further 25 children are now in foster family homes.  Having successfully completed the deinstitutionalisation within its own organisation, HFHK is now able to assist other organisations undertake the deinstitutionalisation process.

The HFHK Alternative Care Management Program provides continued management and support for all the alternative care households that have been established including supervision, regular visits, ongoing training for carers, peer networking, counseling, monitoring, evaluation and review.  Currently there are 5 alternative care households being managed under the HFHK Alternative Care Management Program.
